Medical dermatologic evaluation of skin lesions or medical advice and/or prescriptions for skin conditions are considered office visits, billable to insurance or directly to the patient.
Prior to scheduling a medical evaluation, it is important that you check with your medical insurance provider to ensure that we are in-network. Medical insurance consultation fees vary based on the individual insurance provider. We are contracted with Medicare, Tricare, and most PPO insurances. Unfortunately, are not contracted with HMO insurances.
If you are out of network or do not have insurance, we do offer out-of-pocket pricing for a medical evaluation. Prices for medical evaluations vary based on complexity. This fee does not include the cost of any procedure or treatment done at the time of your evaluation.
All pricing for recommended treatments will be outlined for full transparency during your initial consultation so you can make an informed decision about your skincare and decide on the best payment plan for your finances.
Insurance copays are due at the time of service. Payment for out-of-pocket surgery or procedure is due in full prior to the surgery or procedure.
Please be sure to bring your government-issued ID, Insurance card, and arrive 15 minutes early. For new patients, you will receive a link from Sadio.com to complete your medical history at the time of scheduling.
Cosmetic consultations include discussions about various elective cosmetic (non-insurance covered) procedures that we offer, for example laser, peel, filler, Botox, thread procedures, and aesthetician services. The consultation fee is due at the time of scheduling to secure your appointment. As a courtesy to our patients, your cosmetic consultation fee will then apply to any aesthetic procedure performed within 6 months of your consultation. If treatment is not performed within the 6-month time frame, your consultation fee is payment for the provider’s time. Payment for the consultation is due by credit card at the time of scheduling your appointment. Care Credit cannot be used for this deposit.

Appointment Cancelation and No-Show Policy
Dr. Azi and her office take great pride in providing the best medical care and quality service for all patients. To ensure this is accomplished, the following cancellation and rescheduling policy for all procedure appointments is necessary and strictly enforced.
Our office requires a deposit to be placed at the time of scheduling your cosmetic consultation/procedure. This will be applied to your balance due at the time of the appointment. If you are needing to cancel or reschedule your appointment, we require at least 2-business days’ notice. If our office is provided less than a 2 business days’ notice or you “no-show”, your deposit is non-refundable and non-transferrable, and an additional deposit is required to reschedule as the original deposit is forfeited.
For standard appointments, if you need to cancel or reschedule, we request notice at least 24 hours in advance. Otherwise, there is a $75 cancellation/no-show fee.
Our time and our patient’s time are extremely valuable to us, and we appreciate your understanding.

Medical co-payments must be rendered at the time of the visit. We accept VISA, MASTERCARD, AMERICAN EXPRESS, Debit Cards, Cash, and Checks.
For cash patients, payment in full is due at the time of service. Checks are not accepted for cosmetic services.
We also offer third-party financing through CareCredit® which will allow patients to make payments interest-free for 6 months. APPLY NOW
